Case Manager Position

Position Summary:

As a Case Manager, you'll oversee a designated caseload under the guidance of a Clinical Supervisor and with support from Clinical and Operations Supervisors, including our Clinical Director. You'll serve as the primary liaison for services beyond counseling, supporting clients' needs and facilitating required tasks for comprehensive care.

Primary Responsibilities:

• Advocate and connect clients with community health and social services resources.
• Coordinate internal support to overcome treatment barriers.
• Assist in client screening, intake, and orientation to treatment programs.
• Monitor Medicaid paperwork and facilitate insurance processes for clients.
• Maintain progress notes and essential documentation as directed by our team.
• Collaborate with counselors and external providers for holistic client care.
• Facilitate care coordination and arrange necessary transportation for clients.

Qualifications:

• Ohio Mental Health License: LSW, LPC, LMFT, or Master's Degree.
• 21 years of age or older with reliable transportation and valid Ohio Driver License.
• Knowledge of behavioral management, crisis intervention, DSM V, and clinical procedures.
• Basic computer skills and familiarity with Microsoft Office.
• Experience working with diverse cultural and socio-economic backgrounds.

Tools and Equipment Requirements:

• Proficiency in using phones, computers, and EMR systems.
• Familiarity with Microsoft Office suite and internet browsing.
• Comfortable working in a standard office environment.

Physical Working Conditions:

• Requires a mix of sitting, standing, bending, and occasional lifting.
• Extensive use of computers, keyboards, and office equipment.

Education and License/Certification Requirements:

• Master's degree is required.
• Driver's License is required.
• LSW, LPC, or LMFT certification is preferred.
