Position Overview:
We are seeking a dedicated Ohio Licensed Behavioral Health Professional to serve as a Youth and Adolescent Program Coordinator. This full-time role offers a competitive annual salary ranging from $68,000 to $90,000.

As a Behavioral Health Program Coordinator, you'll play a crucial role in our after-school program, catering to children aged 5-17 years old. Located at 9250 Miles Park Dr., Cleveland, Ohio, this position is at the heart of our commitment to holistic care.
Responsibilities:
- Program Coordination: Oversee and develop policies for our youth and adolescent behavioral health program, ensuring compliance with state regulations.
- Care Coordination: Conduct assessments, coordinate services, and monitor progress, ensuring participants receive tailored support.
- Team Collaboration: Work closely with interdisciplinary teams to ensure integrated care and holistic support.
- Documentation and Reporting: Maintain accurate records and prepare timely reports in accordance with organizational standards.
Essential Duties:
- Support the mental and physical health care of assigned patients, collaborating closely with medical providers and other mental health professionals.
- Screen and assess patients for common mental health and substance abuse disorders, ensuring patient engagement and follow-up care.
- Provide patient education on mental health and substance abuse disorders, explaining available treatment options.
- Systematically track treatment response, monitoring patients for changes in symptoms and treatment side effects.
- Support psychotropic medication management, focusing on adherence, side effects, and treatment effectiveness.
- Provide brief behavioral interventions using evidence-based techniques, overseeing and delivering therapeutic interventions.
- Facilitate in-clinic or outside referrals to evidence-based psychosocial treatments as clinically indicated.
- Participate in regular caseload consultation, communicating treatment recommendations to the patient’s medical provider.
- Track patient follow-up and outcomes using a registry, documenting encounters in the Electronic Health Record (EHR) and other required systems.
- Document patient progress and treatment recommendations for effective communication with the care team.
- Facilitate treatment plan changes for patients needing more specialized mental health care.
- Facilitate referrals for clinically indicated services outside the organization.
- Develop and complete relapse prevention self-management plans for discharged patients.
Qualifications:
- Ohio State Behavioral Health License (LSW, LPC, LMFT) required.
- Familiarity with care coordination models and youth mental health services.
- Experience in behavioral health settings working with youth and adolescents.
- Strong knowledge of mental health disorders and intervention strategies.
- Excellent communication and organizational skills.
- 2-3 years of experience in a mental health care setting.
- Demonstrated ability to collaborate effectively in a team setting.
- Ability to maintain professional relationships with patients and care team members.
- Experience with screening for common mental health and substance abuse disorders.
- Working knowledge of differential diagnosis and evidence-based psychosocial treatments.

Requirements:
- Master's degree in related field (required).
- Reliable transportation and valid Ohio Driver's License.
- Knowledge of crisis intervention and de-escalation methods.
- Proficiency in using computers and relevant software.

- Licensed Mental Health Counselor/Professional Counselor
- Licensed Marriage and Family Therapist
- Licensed Social Worker
- Registered Nurse (BSN recommended)
- Nurse Practitioner
- Licensed Psychologist
- Masters-level licensure candidate/trainee (e.g., LMSW)

Education and Experience Requirements:
