Position Overview: The Community Outreach Advocate is responsible for connecting individuals with serious mental illness to community-based supportive services, addressing social determinants of health, and supporting overall wellness. This role involves working collaboratively with peer recovery specialists to empower individuals in achieving their recovery goals.
You can join us in making Braylon Health the "Go-To" resource for our clients, reducing reliance on emergency rooms and other fragmented medical resources.
Duties and Responsibilities:
- Execute creative outreach strategies to locate and engage individuals from targeted lists.
- Attract potential members to enroll in Braylon Health's Behavioral Health services.
- Explain the benefits of obtaining services at Braylon Health.
- Outreach to families and caregivers.
- Perform telephonic outreach to assigned members, promoting Braylon Health's model of care.
- Connect clients with appropriate resources and conduct home visits.
- Identify barriers to care and gain insight into members' health management.
- Promote Braylon Health's 5-star customer service philosophy in the community.
- Report daily outreach activities into CRM and electronic medical record.
Minimum Qualifications:
- Valid driver’s license, proof of insurance, and reliable personal vehicle.
- Proficient in PC skills, including Microsoft Office.
- Experience in documenting into electronic medical record.
- Knowledge of local community resources.
- Previous experience in human services, community outreach, or medical field preferred.
- High school diploma or equivalent required.
- Ability to work occasional evening and weekend hours when needed.
Preferred Qualifications:
- Bachelors or Masters Degree in social work, psychology, social work,or related human services field.
- SSI/SSDI Outreach Access Recovery (SOAR) certification or willingness to get certified within 6 months.
- Ability to operate a vehicle (company vehicle may be provided), including a driver’s license in good standing.
