What We Do At Braylon Health Wellness Center

Braylon Health Wellness Center is a Community Behavioral Health agency founded in Cleveland, Ohio, that provides counseling, therapy, case management services and primary care to individuals and families. The founders have a background oriented in helping people in need. We envisioned an agency built on engaging the underserved and often overlooked yet vital facet of social service. We maintain active partnerships with community organizations to develop an integrated pool of resources and professionals with the overall objective of improving mental health conditions.

Braylon Health serves children, teens, adults, couples and families with presenting problems that range from situational to serious illnesses.

1. Mental Health Co-occurring assessments and treatment via individual, family, and group therapies
2. Intensive Outpatient Substance Abuse groups lasting 3 hours per session with sessions offered at least 3 days per week (use of the Matrix model as well as Living in Balance)
3. Partial Hospitalization/PH 
4. Case Management Services
5. Re-entry and Drug Court Services
6. Foundation Wellness Program: Outpatient Psychotherapy
7. Peer Support access, individually and via group
8. Psychiatry Services
9. Required monthly client reports, if applicable
10. Group Home & Transitional Housing assistance
11. Mobile Crisis Services
12. Assertive Community Treatment (ACT)
