Overview: Braylon Health, a community-focused behavioral health organization, is seeking a dedicated Substance Use Counselor/Intensive Outpatient Facilitator to join our team. Responsibilities:
- Assess and identify strengths and needs of referred individuals and families.
- Determine substance use disorder level of care and diagnose co-occurring mental health disorders.
- Create comprehensive service treatment plans ensuring linkage to necessary services.
- Provide group and individual counseling to those in Intensive Outpatient Programs.
- Collaborate with referral sources for quick assessments and timely access to treatment.
- Maintain accurate documentation adhering to program guidelines and regulatory standards.
- Participate in program evaluation, cross-systems training, and stay updated on community resources.
- Adhere to ASAM guidelines for treatment planning and documentation.
Minimum Requirements:
- High school diploma or GED; Bachelor's degree or 3 years of relevant experience preferred.
- Current license/certification: LSW, LPC, LISW, LPCC, CDCA, LCDC II, LCDC III, or LICDC.
- Understanding of American Society of Addiction Medicine levels of care.
- Experience in substance use disorder treatment with excellent documentation skills.
- Cultural awareness and ability to communicate effectively.

- Passionate about comprehensive primary care and patient advocacy.
- Experience in Pediatric, Child/Adolescent, Primary Care, or Urgent Care preferred.
- Tech-savvy and adaptable to new workflows and technology.
- Ability to operate in an Internet-based, automated office environment.
